Such errors are usually made by the front-line operator.
The front-line operators are the inheritors of a system's defects.
4.2.17 With these facts in mind and by referring to the Reason model, it can be seen that the actions of other front-line operators were also unsafe and had an influence upon the performance of the flight crew and the outcome of the flight.
